Establishment of Chronic Heart Failure Nurse Clinic Project – WA
Chronic heart failure is a significant burden of disease for patients and cost to the Western Australian (WA) health care system. The prevalence of Chronic heart failure is estimated to be 1–2% of the adult population. The Chronic Heart Failure Nurse Clinic Project was established, with the aims of improving health outcomes for patients managing chronic heart failure in a community setting and improving access to resources and information for primary care nurses.
